INTRODUCTION: Vestibular schwannoma (VS) is the most common tumour of the cerebellopontine angle. The greater accessibility to radiological tests has increased its diagnosis. Taking into account the characteristics of the tumour, the symptoms and the age of the patient, three therapeutic strategies have been proposed: observation, surgery or radiotherapy. Choosing the most appropriate for each patient is a frequent source of controversy. MATERIAL AND METHODS: This paper includes an exhaustive literature review of issues related to VS that can serve as a clinical guide in the management of patients with these lesions. The presentation has been oriented in the form of questions that the clinician usually asks himself and the answers have been written and/or reviewed by a panel of national and international experts consulted by the Otology Commission of the SEORL-CCC. RESULTS: A list has been compiled containing the 13 most controversial thematic blocks on the management of VS in the form of 50 questions, and answers to all of them have been sought through a systematic literature review (articles published on PubMed and Cochrane Library between 1992 and 2023 related to each thematic area). Thirty-three experts, led by the Otology Committee of SEORL-CCC, have analyzed and discussed all the answers. In Annex 1, 14 additional questions divided into 4 thematic areas can be found. CONCLUSIONS: This clinical practice guideline on the management of VS offers agreed answers to the most common questions that are asked about this tumour. The absence of sufficient prospective studies means that the levels of evidence on the subject are generally medium or low. This fact increases the interest of this type of clinical practice guidelines prepared by experts.

RESUMO

The selective photocatalytic oxidation with O2 as oxidant of valencene and thymol was evaluated using nanostructured TiO2 under UV-Vis radiation at atmospheric conditions. The effect of the morphology and optical properties of TiO2 nanotubes and aminate nanoparticles was studied. Different scavengers were used to detect the presence of positive holes (h+), electrons (e-), hydroxyl radicals (•OH), and the superoxide radical anion (O2-) during the photooxidation reaction. Superoxide anion radical is the main oxidizing specie formed, which is responsible for the selective formation of nootkatone and thymoquinone using aminated TiO2 nanoparticles under 400 nm radiation.

Introducción: la hipoacusia neonatal (HN) constituye un problema de salud relevante por su alta frecuencia de presentación y los efectos que esta puede tener en el desarrollo mental y psicológico del paciente. El tamizaje auditivo neonatal (TAN) es fundamental para la identificación de pacientes en riesgo de HN. Objetivo: caracterización de los pacientes con resultados anormales en las pruebas de TAN en el Hospital Universitario Clínica San Rafael. Diseño: estudio observacional descriptivo de corte transversal. Materiales y métodos: se identificaron características biológicas y clínicas en pacientes con emisiones otoacústicas o potenciales evocados auditivos anormales realizados entre 2018 y 2020. Los pacientes incluidos se dividieron en 2 grupos, según la presencia de los factores de riesgo para HN. Resultados: 9027 pacientes se tamizaron, 223 (24,7 %) tuvieron resultados anormales y 19 pacientes se excluyeron. En total, 204 pacientes se incluyeron, de los cuales 46 (22,5 %) fueron de alto riesgo (AR) y 158 (77,5 %) de bajo riesgo (BR). El TAN en pacientes BR se realizó antes del primer mes de vida en 78,5 % de los casos y en 21,4 % de los AR. Las características más frecuentes fueron la ventilación mecánica (45,6 %), la hospitalización en cuidados intensivos (43,5 %), la anormalidad craneofacial (15,2 %) y la hiperbilirrubinemia mayor de 20 mg/dL (10,8 %). Conclusiones: se deben optimizar los esfuerzos y recursos para realizar las pruebas de tamizaje auditivo antes del primer mes de vida al 100% de pacientes. Se deben realizar estudios complementarios para identificar los diagnósticos auditivos finales de los pacientes con resultados anormales, así como el tipo de tratamiento y la rehabilitación auditiva recibida.


Introduction: Neonatal hearing loss (NHL) is a relevant health problem due to its high incidence and the effects it may have both on the mental and psychological development of the patient. Neonatal hearing screening (NHS) is essential for the identification of patients at risk of NHL. Objective: To identify the characteristics of patients with abnormal NHS test results at the Hospital Universitario Clínica San Rafael. Design: Cross-sectional descriptive observational study. Materials and methods: Biological and clinical characteristics were identified in patients with abnormal otoacoustic emissions and/or auditory evoked potentials (AEP) performed between 2018 and 2020. Patients involved were divided into 2 groups according to the presence of risk factors for NHL. Results: 9027 patients were screened, 223 (24.7%) had abnormal results, but 19 patients were excluded. 204 patients were included in total, 46 (22.5%) of which were high risk (HR) and 158 (77.5%) were low risk (LR). NHS in LR patients was performed before the first month of life on 78.5% of cases and 21.4% of HR patients. The most frequent characteristics were mechanical ventilation (45.6%), intensive care unit admission (43.5%), craniofacial abnormality (15.2%) and hyperbilirubinemia greater than 20 mg/dL (10.8%). Conclusion: Efforts and resources should be optimized as means to perform hearing screening tests before the first month of life on 100% of patients. Complementary studies should be performed with the aim to identify the final auditory diagnoses of patients with abnormal results, as well as the type of treatment and auditory rehabilitation offered.

A 10 year-old endurance Arabian mare was presented with an invasive, firm, multilobulated mass at the left ventral face of the tongue. The mare had a history of four months of ptyalism, dysphagia, severe halitosis, and dysmastication. The Evaluation of the horse included physical and oral examination, complete blood count and serum biochemistry profile, computed tomography evaluation of the head, and histopathology of the mass. The computerized tomography (CT) head scan showed the homogeneous mass in the oral cavity confirming the degree of invasion on the tongue, as well as a small mineral focus at the left dorsal mandibular border and increased density of the submandibular lymph nodes. At necropsy, gross findings in the oral cavity revealed an irregular, nodular and partially ulcerated mass at the level of the body of the tongue in the ventral aspect, with a size of approximately 10 × 15 × 8 cm. Histologically, the tongue revealed a neoplastic process of mesenchymal muscle origin. The tumor cells showed distinctive morphological and architectural patterns in some areas. An immunohistochemistry panel was done yielding a positive reaction for Calponin and Desmin, confirming diagnostics of as a soft tissue leiomyosarcoma in the tongue. Immunohistochemistry results, in combination with histopathologic morphology, were suggestive of a leiomyosarcoma originating in the perivascular wall tissue. Oral leiomyosarcoma are very rare in veterinary medicine, and to the authors' knowledge, this is the first case report to utilize histopathology, immunohistochemistry and imaginology to describe oral leiomyosarcoma in a horse.

Coronavirus disease 2019 (COVID-19) is currently the major public health problem worldwide. Neutral electrolyzed saline solution that contains reactive chlorine and oxygen species may be an effective therapeutic. In the present study, the treatment efficacy of intravenous and/or nebulized neutral electrolyzed saline combined with usual medical care vs. usual medical care alone was evaluated in ambulatory patients with COVID-19. A prospective, 2-arm, parallel-group, randomized, open-label, multi-center, phase I-II clinical trial including 214 patients was performed. The following two outcomes were evaluated during the 20-day follow-up: i) The number of patients with disease progression; and ii) the patient acceptable symptom state. Serial severe acute respiratory syndrome coronavirus 2 naso/oro-pharyngeal detection by reverse transcription-quantitative (RT-q) PCR was performed in certain patients of the experimental group. Biochemical and hematologic parameters, as well as adverse effects, were also evaluated in the experimental group. The experimental treatment decreased the risk of hospitalization by 89% [adjusted relative risk (RR)=0.11, 95% confidence interval (CI): 0.03-0.37, P<0.001] and the risk of death by 96% (adjusted RR=0.04, 95% CI: 0.01-0.42, P=0.007) and also resulted in an 18-fold higher probability of achieving an acceptable symptom state on day 5 (adjusted RR=18.14, 95% CI: 7.29-45.09, P<0.001), compared with usual medical care alone. Overall, neutral electrolyzed saline solution was better than usual medical care alone. Of the patients analyzed, >50% were negative for the virus as detected by RT-qPCR in naso/oro-pharyngeal samples on day 4, with only a small number of positive patients on day 6. Clinical improvement correlated with a decrease in C-reactive protein, aberrant monocytes and increased lymphocytes and platelets. Cortisol and testosterone levels were also evaluated and a decrease in cortisol levels and an increase in the testosterone-cortisol ratio were observed on days 2 and 4. The experimental treatment produced no serious adverse effects. In conclusion, neutral electrolyzed saline solution markedly reduced the symptomatology and risk of progression in ambulatory patients with COVID-19. The present clinical trial was registered in the Cuban public registry of clinical trials (RPCEC) database (May 5, 2020; no. TX-COVID19: RPCEC00000309).

La cirugía electiva en otología se ha visto afectada por la pandemia del COVID-19. No sabemos cuánto tiempo pasará hasta que veamos un descenso en el número de casos nuevos y podamos volver al ejercicio médico que conocíamos. Mientras tanto, debemos adaptarnos rápidamente a la nueva forma de funcionamiento de los servicios quirúrgicos en otología, siguiendo los protocolos que están surgiendo para la reactivación gradual y segura de esta clase de actividad. El objetivo de este artículo es realizar una revisión narrativa de la literatura sobre las nuevas recomendaciones para la reactivación de los servicios quirúrgicos otológicos durante esta pandemia, estableciendo pautas seguras en el manejo perioperatorio. Una de las metas más importantes es brindar una guía sobre la forma de comportarse dentro del ambiente de los quirófanos, sus cambios operacionales, el correcto uso de elementos de protección personal y dar a conocer métodos que busquen disminuir la exposición del equipo quirúrgico a la aerosolización, describiendo una técnica novedosa de vestido del microscopio de doble cubrimiento con extensión cefálica para el cirujano.


Otologic surgical procedures have been affected by the COVID-19 pandemic. Is not known how long it will be until we see a decrease in the number of new cases and when we will be able to return to our usual medical practice. Meanwhile, we must adapt quickly to a new surgical practice in the COVID era. This article discuses recommendations for reactivating otological surgical services during the pandemic, establishing safe guidelines for ear surgery and for postoperative patient follow-up. One of the most important goals is to provide recommendations on how to behave in the operating room, how to use personal protection elements and to present new methods that seek to reduce the exposure of the surgical team to aerosolization. Here we describe a novel technique using the already described double draping technique and adding an extension for the surgeon´s protection.

Objetivo: describir la experiencia de los pacientes llevados a una estapedotomía en el Hospital Universitario Clínica San Rafael en los años 2018 y 2019. Materiales y métodos: estudio descriptivo de corte transversal, en el que se revisaron un total de 32 historias de pacientes intervenidos con una estapedotomía entre los años 2018 y 2019 en el Hospital Universitario Clínica San Rafael. Resultados: entre los años 2018 y 2019 se realizaron 32 estapedotomías, 62 % en mujeres, con una media de edad de 48,1 años, el 54,9 % fueron en el oído derecho; al 84,3 % se le realizó un abordaje transcanal, en el 78,1 % se usó prótesis Clip-pistón, en el 46,8 % se usaron prótesis de 4,75 mm; en el 69,7 % de los casos presentaban hipoacusia moderada preoperatoria, 6 pacientes presentaron vértigo, solo en 1 paciente se presentó Gusher como complicación y 1 perforación timpánica; en el 100 % de los pacientes se encontró un cierre satisfactorio del GAP de la vía aérea y ósea. Conclusión: la estapedotomía es un procedimiento que se realizó de manera exitosa en el Hospital Universitario Clínica San Rafael durante los años 2018 y 2019, teniendo en cuenta los buenos resultados auditivos posoperatorios dados por el cierre de la brecha entre la vía aérea y la vía ósea.


Objective: describe the experience of stapedotomy patients at the Hospital Universitario Clínica San Rafael in the years 2018 and 2019. Material and method: descriptive study in which a total of 32 medical records of patients undergoing stapedotomy between 2018 and 2019 at the Hospital Universitario Clínica San Rafael were reviewed. Results: between 2018 and 2019, 32 stapedotomies were registered, 62% female, with a mean age of 48.1 years, 54.9% in right ear; 84.3 % with transcanal approach, 78.1% Clip-piston prostheses were used, in 46.8 % 4.75 mm prostheses were used; 69.7 % presented moderate preoperative hearing loss, 6 patients presented vertigo, only 1 patient presented a gusher complication and 1 tympanic perforation. 100 % of the patients had a satisfactory air-bone gap closure. Conclusion: stapedotomy is a successful procedure performed at the Hospital Universitario Clínica San Rafael during 2018 and 2019, considering the excellent postoperative auditory results given by the air-bone gap closure.

RESUMO

La pandemia por COVID-19 representa un desafío para la atención médica mundial. Esto se debe, principalmente, a las características del virus y a la ausencia de evidencia clínica para afrontar sus consecuencias. Teniendo en cuenta que la especialidad de otorrinolaringología es una de las que se encuentra en mayor riesgo de exposición, es importante establecer recomendaciones para su práctica segura. El uso de equipos de protección personal (EPP) se deberá realizar de acuerdo con las características del trabajador de la salud y la actividad que deba desarrollar. Así mismo, la infraestructura de los consultorios y áreas de procedimientos deberá contar con las adecuaciones necesarias para mantener el distanciamiento social. Los métodos para la desinfección de superficies, instrumentos y equipos se deberán realizar con las sustancias recomendadas, según el material y las características de cada una de estas. La práctica segura y eficiente permitirá la reactivación de los servicios de atención médica.


The COVID-19 pandemic represents a challenge for global healthcare. This is mainly due to the virus' characteristics and the absence of clinical evidence to face its consequences. Considering that otolaryngology has a high risk of exposure, it is important to establish recommendations for its safe practice. The use of personal protective equipment (PPE) must be carried out according to the healthcare professional's characteristics and specific activity. Likewise, the infrastructure of the office and office-based procedure areas must have conditions to maintain social distancing. The methods for disinfecting surfaces, instruments and equipment must be carried out with the recommended substances according to its materials and characteristics. A safe and efficient medical practice will help to reactivate healthcare services.

INTRODUCTION: Wilkie's syndrome is a rare entity that presents as a chronic intestinal obstruction secondary to mechanical compression of the third portion of the duodenum due to a decreased aorto-mesenteric angle. This entity has a reported incidence of 0.012 to 2.4% and can lead to high morbidity and mortality; In addition, there are major controversies regarding its diagnosis and treatment, which is why the authors present a case report of a patient with a diagnosis of Wilkie's syndrome and its surgical management via laparoscopy. METHODS: A case of a patient with clinical and imaging manifestations of superior mesenteric artery syndrome is presented, along with acritical review of the literature, taking into account articles published in the PubMed and ScienceDirect databases during the past 8 years. CLINICAL CASE: A 27-year-old patient with 20 years of abdominal pain that exacerbated one week before admission, with a clinical and imaging manifestations of Wilkie's syndrome. Patient presents refractoriness to medical management, which led to a surgical approach with laparoscopic duodenum-jejunostomy that was carried out without perioperative complications and satisfactory results. DISCUSSION AND CONCLUSIONS: The superior mesenteric artery syndrome is a rare entity that in some scenarios requires surgical management. The laparoscopic duodeno-jejunostomy is the procedure of choice to manage this entity as evidenced in our case, which is also consistent with what is reported in the world medical literature.


INTRODUCCIÓN: El síndrome de Wilkie es una entidad poco frecuente que se presenta como una obstrucción intestinal crónica secundaria a la compresión mecánica de la tercera porción del duodeno; la compresión es extrínseca por una disminución del ángulo entre la arteria mesentérica superior y la aorta. Esta anomalía tiene una incidencia informada en la bibliografía médica mundial de 0.012 a 2.4% y puede propiciar una elevada morbimortalidad; de forma adicional, existen grandes controversias en relación con su diagnóstico y tratamiento, por lo cual los autores presentan un informe de caso de un paciente con diagnóstico de síndrome de Wilkie y su manejo quirúrgico por vía laparoscópica. MÉTODOS: Se presenta un caso de un paciente con diagnóstico clínico e imagenológico de síndrome de arteria mesentérica superior y se realiza un revisión crítica de las publicaciones, con atención a los artículos publicados en las bases de datos de PubMed y ScienceDirect en los últimos ocho años sobre el síndrome de Wilkie y se completa la revisión con artículos clásicos sobre el tema. CASO: Paciente de 27 años con 20 años de dolor abdominal agudizado una semana antes del ingreso, con diagnóstico clínico e imagenológico de síndrome de Wilkie. El enfermo muestra resistencia al tratamiento médico, por lo que se practicó una duodenoyeyunostomía laparoscópica sin complicaciones perioperatorias y con resultados satisfactorios. DISCUSIÓN Y CONCLUSIONES: El síndrome de arteria mesentérica superior es un trastorno raro que exige medidas quirúrgicas en algunos escenarios. La duodenoyeyunostomía laparoscópica es el procedimiento de elección para tratar esta entidad, como se evidenció en este caso, lo cual concuerda con lo publicado en la bibliografía médica mundial.

The first example of photoredox catalyzed difluoromethylation of unactivated alkenes coupled with C-C bond formation to an aryl ring is reported. The reactions are conducted under mild conditions and afford tetralin derivatives bearing difluoromethyl as well as other fluoroalkyl groups in good to high yields. In addition, the study indicates that 6-exo radical cyclization of an alkyl radical to a phenyl ring is faster than the respective 5-exo radical cyclization. A computational study provides insights to the experimental results.

The origins of regio- and stereoselectivity in the ring-opening metathesis polymerization of 3-substituted cis-cyclooctenes by monoaryloxide pyrrolide imido alkylidene (MAP) Mo- and W-based catalysts are determined at the M06-2X/SDD|6-311+G(2df,p)//M06-L/SDD|6-31G(d)|MIDI! level of density functional theory. Considering cis-cyclooctene (COE) and 3-methyl-cis-cyclooctene (3MCOE) as monomers and W(N-t-Bu)(CH-t-Bu)(OHMT)(Pyr) (OHMT = hexamethylterphenoxide, Pyr = pyrrolide) as a catalyst, all possible syn and anti combinations of alkylidene and cyclic olefin, relative to the imido ligand, are evaluated. The observed Z-selectivity for the ring-opening metathesis (ROM) of COE is due to the large size of the aryloxide ligand, which forces both the alkylidene and the incoming cyclic olefin to be syn relative to the imido ligand. As determined previously for Grubbs' second-generation catalyst (G2), breakdown of the metallacyclobutane intermediate is the rate-limiting step for cyclic olefins having ring sizes exceeding five carbon atoms. Contrary to the G2 case, however, the ring-opening of 3MCOE by MAP catalysts prefers a proximal (3-substituent closest to the metal center) over a distal (3-substituent furthest from the metal center) approach. In all calculated paths, we observe inversion of catalyst configuration after each catalytic cycle.

Under specific high concentration, high temperature conditions, methyl 2,2-difluoro-2-(fluorosulfonyl)acetate (MDFA) has been found to act as a very efficient source of difluorocarbene, exhibiting carbene reactivity characteristics comparable to those exhibited by trimethylsilyl 2,2-difluoro-2-(fluorosulfonyl)acetate (TFDA). For example, in reaction with highly unreactive n-butyl acrylate and using only 2 equiv of MDFA, a yield of 76% of difluorocyclopropane product was obtained after 2 days.

The product of S(N)Ar addition of the enolate of ethyl acetoacetate to perfluoro[2.2]para-cyclophane exists entirely as its enol tautomer 5. This enol exhibits two NMR signals for its enolic proton, and these signals were shown to derive from the presence of two, equal energy conformations that were observable as distinct, stable conformations at room temperature, but which when heated, interconverted with an energy barrier of 23.5 kcal mol(-1). These atropisomers were characterized by NMR, with details of this analysis being provided. Computational work corroborated the NMR conclusions, and provided additional insight into all structural, thermodynamic and kinetic results. Enol product 5 was cyclized, under basic conditions, to form a benzofuran product 6. Its structure was confirmed by NMR, with further structural and mechanistic insights being provided by calculations.

The Friedel-Crafts reactions of 2,2-difluorocyclopropanecarbonyl chloride with various arenes did not lead to the straightforward formation of the expected aryl 2,2-difluorocyclopropyl ketones. Instead the reactions proceeded, to various degrees depending on the reactivity of the arene, via an apparent rearrangement of the initially formed acylium ion to form novel aryl 3-chloro-3,3-difluoropropyl ketones. The ring-opened product was formed exclusively, and therefore the reaction may be synthetically useful when relatively unreactive arene substrates such as benzene, toluene, and p-xylene are used. No conditions were found where ring-intact products could be formed exclusively when using substituted benzenes as substrates, with the very reactive substrate thiophene being most selective in that regard, favoring ring intact product 3 with a selectivity of 98:2. The regioselectivity of ring-opening was examined and compared with other related systems computationally.

In the process of studying the chemistry of perfluoro[2.2]paracyclophanes (PFPCs), a novel class of compounds, it became necessary to identify some disubstituted products. To achieve this goal, we characterize in this work some monosubstituted PFPCs, identifying their (19)F-(19) F coupling patterns, and establishing a methodology for the assignment of their (19)F chemical shifts. The pattern of coupling constants indicates a skewed geometry in which the upper deck moves towards or away from the substituent, depending on the substituent electron-donor character and size. Quantum chemical calculations, performed at the HF/6-311 + G(d,p)//B3LYP/EPR-III level of theory, confirmed the conformations inferred from coupling constants and reproduced well the values of the couplings. Transmission mechanisms for the FC term of four- and five-bond (19)F-(19) F couplings are discussed in detail. Understanding the conformational preferences of PFPCs and how they are reflected by the coupling constants facilitates the assignment of (19)F chemical shifts in monosubstituted PFPCs and the identification of the disubstituted products.

Conformational equilibria in novel C-nitroso derivatives of indolizines and 3- and 5-azaindolizines have been studied by NMR. (13)C chemical shifts of the carbon alpha to the nitroso group confirmed that these compounds are present in solution as monomers. The conformers arising from restricted rotation about the C-NO bond in monomers were identified by the chemical shifts of the carbon beta to the nitroso group. Barriers to rotation in these compounds were unusually high, particularly for substituents in position 3 of indolizine. Ethyl 2-(methylamino)-1-nitrosoindolizine-3-carboxylate displayed conformers arising from the restricted rotation about the C-COOR bond. Molecular modelling demonstrated that in 1-nitrosoindolizines, the position of the conformational equilibrium is due to steric effects, while for 3-nitrosoindolizines electronic effects prevail.

Objetivo: Se buscó delimitar y explicar los factores psicológicos inherentes en la no práctica, práctica y mantenimiento de la actividad física en la población estudiantil de la Universidad Nacional de Colombia, en el año 2004. Métodos Se empleó el modelo del Proceso de Adopción de Precauciones (PAP) para comprender la conducta de la actividad física como proceso y estado, desde el sedentarismo hasta la formación de estilos de vida activos. Se realizó la adaptación del modelo PAP para la explicación de la conducta en cuestión, en donde se sopesaron los factores psicológicos a través de cada una de las etapas de cambio conductual. Se valida la adaptación lograda mediante el diseño y aplicación de un instrumento psicométrico que evalúa las tendencias de práctica, no práctica y mantenimiento de actividad física en estudiantes de la Universidad Nacional de Colombia. Resultados La aplicación del instrumento devela la influencia y relación de los factores psicológicos que intervienen directamente en las etapas del modelo propuesto. Conclusiones El modelo es un sólido constructo teórico-metodológico, que da cuenta de forma adecuada de los factores psicológicos que intervienen en la no práctica, práctica y mantenimiento de la actividad física. En este sentido el modelo puede ser utilizado en la creación de una estrategia de intervención en promoción de la salud y prevención de enfermedad a partir de estilos de vida activos.

Conclusions The model was a solid theoretical and methodological construct, suitably expressing all psychologist features intervening in the non-practice, practice and consolidation of physical activity. The model can thus be used in creating an intervention strategy related to promoting health and preventing disease through encouraging active lifestyles.

RESUMO

This is a case report of a 19 year-old woman, with an acute recurrent pancreatitis diagnose, and pancreas divisum as anatomic alteration. An extensive evaluation including specialized labs, ERCP was done. The diagnosis is suspected during ERCP and confirmed by minor papilla injection. Treatment is directed towards relieving outflow obstruction at the level of the minor papilla, with successful results.

PURPOSE: The purpose of this prospective multicenter study was to evaluate the efficacy of 3i threaded implants for the treatment of edentulous patients in a 1- to 5-year period. This article reports the total data and global results of 3 threaded designs of 3i implants: self-tapping, ICE, and Osseotite. MATERIALS AND METHODS: A total of 1,583 implants (619 ICE, 545 Osseotite, and 419 self-tapping) were placed between 1995 and 1999 in 528 patients at 13 European clinical centers. The average age of the patients was 53.6 years. Clinical and radiographic evaluations were performed annually for up to 5 years. RESULTS: Of the total implants, 707 were placed in the maxilla and 876 in the mandible. A total of 1,162 implants were placed in posterior segments. Forty-eight implants were lost to follow-up and 55 were failures. The most frequent prosthetic indication was the short-span fixed prosthesis (440 cases), followed by 172 single-tooth replacements, 56 long-span prostheses, and 4 overdentures. Radiographic evaluation after 6, 12, and 24 months of implant loading showed, respectively, mean crestal bone loss of 0.04 +/- 1.3 mm, 0.12 +/- 1.6 mm, and 0.2 +/- 1.7 mm. A cumulative survival rate of 96.5% was observed 5 years after implant placement, with 97.2% survival in the maxilla and 95.8% in the mandible. The survival rate was similar in anterior (96.7%) and posterior (96.5%) segments. DISCUSSION: A total of 55 failures were reported in this study with 47 early failures and 8 late failures. The rate of late failures is of utmost importance for the restorative dentist. CONCLUSION: This clinical study gives evidence of very high success rates using 3 threaded designs of 3i implants.
